Case Management Program

PromiseCare provides Case Management Services to our members.

The purpose of this program is to assist in the coordination of resources and to create flexible, cost-effective options for catastrophically or chronically-ill members on a case-by-case basis and to assist in facilitating quality care with individualized goals and improved functional outcomes.

This may include, but is not limited to, patients who are:

  • Medically fragile
  • Polypharmacy users or abusers
  • Non-compliant members
  • High risk for hospitalization
  • Mentally or physically disabled
  • Experiencing a medical condition requiring a complex medical regime
  • Experiencing a medical condition requiring an extended period of treatment
  • Experiencing a terminal illness
  • Needing organ transplants

You can request a referral or refer a patient to case management by calling 951-390-2840 and speak to a customer service representative who will assist in completion of your referral.
